Welcome to the Outer East Health & Community Support Alliance website.
The Outer East Health and Community Support Alliance (Outer East Alliance) is a voluntary alliance of agencies from the municipalities of Knox, Maroondah and Yarra Ranges, that are committed to collaborative thinking, planning and action, which improves the experiences and outcomes of people who access the primary care system.
The Outer East Alliance aims to improve and sustain the quality of life of residents in the Outer East of Melbourne, and the health and wellbeing of the community as a whole. The Outer East Alliance recognises that the functional coordination of services is a key factor in providing high quality and responsive services to the community, particularly for consumers with complex and multiple needs. It also recognises that effective links and collaboration between primary care agencies is fundamental to the achievement of an integrated service system that promotes streamlined access and continuity of care for consumers.
In April 2000, the Department of Human Services released its Going Forward: Primary Care Partnerships Strategy/Program. The Primary Care Partnerships (PCP) program aims to improve the health and wellbeing of consumers through an integrated, cooperative and coordinated approach to service planning and delivery within the community health and primary care services sector. The Outer East Alliance is the auspice body for the Outer East PCP, promoting collaboration and best practice in service planning and delivery to ensure efficient, coordinated and consumer focused services.
This site is designed as a mechanism to share information and facilitate communication regarding primary health care and PCP activity in the Outer East.
